Scroll down to the REQUEST AN ABSENTEE BALLOT section and follow the prompts there.  

Your Town Clerk will receive a notice that you applied and will mail the ballot to you.

You can track the status of your Absentee ballot at https://myvote.wi.gov/en-us/My-Ballot where you will see when the post office received the ballot from your Town Clerk for delivery to you by selecting the Track My Ballot tile.  When you mail your completed ballot back to your Town Clerk, she will post the date that it was received.
